"Client Waivers and Questionnaires" is a feature that offers the ability to complete a paperless form gathering real time data from your new and existing clients. This is useful for health forms, PAR-Q, and agreements. You can set questions which your clients will need to answer and acceptance statements with tickboxes they have to accept.
To set this up, please follow these steps:
1) Go to your Client Area
2) Expand "Online Client Waivers/Questionnaires"
3) Tick "Enable Client Area questions for new clients signup?"
4) Use "Add" button to create your questions and/or statements
5) You can drag and drop items to set desired order.
6) And that's it. Your form is ready to be used!
HOW IS THIS DISPLAYED ON Client Area?
HOW COMPLETED FORM IS STORED ON YOUR CLIENT PROFILE?
For your existing clients that hasn't filled up 'Waives and Questionnaires', you can simply go into their profile to complete the form. You can do this following these steps:
1) Go into 'Client' tab
2) Pull up your client Profile
3) Head to 'Waivers/Questionnaires' subtab
4) Click 'Fill in Questionnaire for <client name>' button
